PCA 101: Foundations - Primary Care Advancement for Population Health and Care Management

This course provides an awareness-level orientation to Population Health and Care Management within NCQA’s Primary Care Advancement program. Learners are introduced to population health as a practical approach for understanding patient panels, identifying care gaps, supporting risk-based outreach and coordinating services across the care journey.

The course emphasizes shared language, foundational care management concepts, common workflows, team roles, data-informed outreach and connections to improved outcomes, patient experience, care team experience, cost and value-based care performance. Learners use foundational examples to recognize strengths, gaps and practical next steps. The course does not teach learners how to build a comprehensive population health strategy, conduct advanced risk stratification or design full care management workflows.

What You Will Learn

By the end of the course, participants will be able to:

• Define foundational population health and care management concepts within Primary Care Advancement.
• Explain how panel management, risk awareness and care gap identification support outcomes, patient and care team experience, cost and value-based care performance.
• Recognize foundational care management capabilities related to patient identification, outreach, care planning, referral follow-up, documentation, team roles and basic data use.
• Use a simple population health snapshot to identify current-state strengths, unclear areas and gaps.
• Prioritize foundational improvement initiatives and draft a brief 6–18 month direction for next steps while recognizing that comprehensive care management design is addressed in more advanced learning.

Continuing Education
