PCA 101: Foundations - Primary Care Advancement for Network Partnerships
This course provides an awareness-level overview of Network Partnerships in NCQA’s Primary Care Advancement program. Learners explore how primary care teams use partner relationships to connect patients with services and supports outside the practice. The course focuses on partner types, referral and communication basics, information return, follow-up and use of a basic partner inventory to understand the network and identify gaps.
What You Will Learn
By the end of the course, participants will be able to:
- Identify common partner types and how networks help close care gaps.
- Describe basic referral, communication and shared expectation practices.
- Explain how a basic partner inventory helps practices understand commonly used partners.
- Recognize patient barriers that may affect referral follow-through and information return.
