PCMH 101: Foundations - Mastering Care Coordination and Transitions in the PCMH Model
This course introduces learners to the Care Coordination and Transitions concept within the PCMH model. Learners explore how practices systematically track diagnostic tests, manage referrals, coordinate with specialists, share information across care settings, and support patients during transitions. The course emphasizes documentation, evidence of implementation, patient safety, timely communication, and effective use of systems and relationships to support coordinated, high-quality care.
What You Will Learn
At the conclusion of the webinar participants will be able to:

Describe the purpose of care coordination and care transitions in supporting patient safety, communication, and cost-effective care.

Identify required components of a process for tracking lab and imaging tests, including overdue results, abnormal findings, clinician review, and patient notification.

Explain how practices manage referrals by sending complete information, tracking referral status, reconciling specialist reports, and following up on overdue responses.
