PCMH 101: Foundations - Mastering Team-Based Care (TC) for Sustainable Transformation
Learners examine the three competencies and related core and elective criteria, including leadership and staff responsibilities, patient and caregiver participation, care team communication, staff involvement in quality improvement, behavioral health coordination, and communication with patients. Practical examples help learners recognize appropriate processes and evidence for demonstrating implementation.

Differentiate the purpose and evidence expectations of Competencies A, B, and C.

Apply the criteria to a practice scenario to identify actions that strengthen coordination, quality improvement, and patient partnership.
