Diabetes Recognition Program 2026: Advancing the Delivery of High-Quality Diabetes Care

The Diabetes Recognition Program recognizes clinicians who use evidence-based measures to provide quality care to their patients with diabetes. To earn NCQA Recognition, clinicians submit data demonstrating that they meet specific criteria for 9 clinical performance measures and 1 utilization measure comprising the DRP. This education course is designed as a deep dive into the Diabetes Recognition Program, what the program is, why it matters, and how it works. The course includes two modules of the Diabetes Recognition Program:

• Module 1: Understanding the Diabetes Recognition Program
• Module 2: Diabetes Recognition Program Q-PASS Demonstration

What You Will Learn

At the conclusion of the course, participants will be able to: 
Recognize the importance of evaluating diabetes care. 
Identify the core clinical benefits of the Diabetes Recognition Program.
Describe processes and resources to achieve Diabetes Recognition.
Interpret the required measure set and scoring.
Describe the patient selection methodology and data submission process.
Outline the features and functions of the Q-PASS system.

Who Should Attend

This course is designed for any health care professional, health care decision maker or consultant interested in getting the inside perspective on the Diabetes Recognition Program directly from expert faculty from NCQA. This course is also geared towards MDs, DOs, APRNs or PAs and care managers treating diabetic patients, as well as ABFM Board Certified Physicians.

Caroline Blaum

Caroline Blaum, MD

Dr. Caroline S. Blaum joined NCQA from NYU Langone Health where she was the Diane and Arthur Belfer Professor of Geriatric Medicine and Director of the Division of Geriatrics and Palliative Care from 2012-2020. At NYU, Dr. Blaum maintained an active clinical practice and supervised clinical education for medical trainees in geriatrics and palliative care.  She led an extensive reach research program in translational research that was supported by the NIA, AHRQ, PCORI, and The John A Hartford Foundation concerning models of care for vulnerable populations, multiple chronic conditions, frailty, and diabetes in older adults. Prior to going to NYU, Dr. Blaum was Professor and Assistant Dean for Population Health at the University of Michigan Health System where she led UM’s experiences in Medicare managed care, ACO’s and PCMH. Dr. Blaum has been active in national policy work related to delivery system redesign and quality of care for older adults and people with multiple chronic conditions and has worked with the American Geriatrics Society, the AMA PCPI, and NQF.

 

Tonna Dexter

Assistant Director, Recognition Operations

Tonna Dexter MSN, RN, MBA, CPHQ, LNC, PCMH CCE is Assistant Director of Recognition Operations at NCQA. Her responsibilities include assisting staff with NCQA Recognition programs and providing support to practices going through the recognition process. Prior to NCQA, Tonna has spent over a decade mentoring providers and healthcare organizations, guiding them through quality incentives, process improvement,  and accreditation.

Tonna holds an MSN in Nursing Executive Leadership from Chamberlain University and an MBA from Keller Graduate School of Management. Tonna has been an NCQA Patient-Centered Medical Home Certified Content Expert (PCMH CCE) since 2023.

 

Kristen Bishop

Kristen Bishop
Senior Health Care Analyst, Performance Measurement
NCQA

Kristen Bishop, BS, is a Senior Health Care Analyst in the Performance Measurement department at NCQA. Ms. Bishop earned her Bachelor of Science in Public Health and has interest in chronic conditions research as well as health equity.
