PCMH 301: Advanced - Care Management Excellence - A Systems Approach
Effective care management is not defined by enrollment numbers, risk scores or completed care plans. High-performing organizations design care-management systems that identify the right patients, connect them to the right interventions, measure what matters to the patient and sustain outcomes over time.
PCMH 301: Care Management Excellence – A systems Approach is an advanced course that explores how successful practices move beyond compliance to build strategic, patient-centered care-management programs. Learners will examine the interrelationships among patient identification, risk stratification, care planning, care coordination, person-centered outcomes, team workflows and quality improvement.
Using realistic practice scenarios, care plans, patient cases and operational data, participants will evaluate care-management approaches, critique program design decisions and develop strategies for creating sustainable, high-performing care-management systems.
This course emphasizes systems thinking, operational decision-making and practical application of PCMH Care Management standards within real-world practice environments.
What You Will Learn
At the end of the course learners will be able to:
• Evaluate a practice’s care management population and risk-stratification approach to determine whether patient need, program intent, staffing capacity and operational resources are appropriately aligned.
• Redesign care plans and supporting workflows to promote collaborative, person-centered care; meaningful goals; self-management; barrier resolution; and continuity across settings.
• Select and interpret clinical, utilization, engagement and patient-reported outcome measures to assess whether care management is producing meaningful improvement for patients and the practice.
• Develop and defend an integrated care management improvement strategy that connects CM requirements with related TC, KM,CC and QI workflows while addressing feasibility, sustainability, and performance.
