Join NCQA PCMH program operations staff and leaders, Recognized practices and PCMH Certified Content Experts for the 2025 PCMH Office Hours recorded on June 25, 2025 

As an organization committed to advancing quality improvement and the patient-centered medical home model of care, NCQA invites you to view a special PCMH Office Hours session that will foster a deeper understanding of program requirements and help learners clarify challenging criteria when they view the recorded event. 
  
The webinar discussion addressed the topics below and more: 


•    Custom measures.
•    Quality improvement.
•    Care management.
•    Corrective action plans.

On-demand viewers also receive the post event resources, and PCMH Certified content experts still have the opportunity to earn additional MOC credit when they participate in the post event activity by July 31, 2025.

Faculty

Jennifer Anglin

Jennifer Anglin, DHSc, PCMH CCE

Jennifer Anglin is a Senior Policy Manger of Recognition Policy at NCQA. Her responsibilities include creation and maintenance of PCMH resources and providing support to practices going through transformation or annual review. Prior to NCQA, Jennifer worked for Purdue University by assisting practices throughout the state of Indiana on quality improvement and research projects as well as supporting them to meet Medicaid’s Promoting Interoperability program and Medicare’s MIPS program. Jennifer’s professional interests include health policy, research, and education. She has a particular interest in social determinants of health and the behavioral health aspect of Patient-Centered Medical Homes.  

Jennifer holds a BA in secondary education from Valparaiso University and a master’s degree in health science from Indiana State University. Jennifer is a Certified Health Education Specialist and has been a NCQA Patient-Centered Medical Home Certified Content Expert (PCMH CCE) since 2013.

 

Tonna Dexter

Tonna Dexter MSN, RN, MBA, CPHQ, LNC, PCMH CCE

Tonna Dexter MSN, RN, MBA, CPHQ, LNC, PCMH CCE is Assistant Director of Recognition Operations at NCQA. Her responsibilities include assisting staff with NCQA Recognition programs and providing support to practices going through the recognition process. Prior to NCQA, Tonna has spent over a decade mentoring providers and healthcare organizations, guiding them through quality incentives, process improvement,  and accreditation.

Tonna holds an MSN in Nursing Executive Leadership from Chamberlain University and an MBA from Keller Graduate School of Management. Tonna has been an NCQA Patient-Centered Medical Home Certified Content Expert (PCMH CCE) since 2023.

 

Jessica Thomas

Jessica Thomas, DrPH

Jessica Thomas joined NCQA in January 2025 as Assistant Vice President of Evaluation Product Delivery. In her role, she is focused on providing strategic direction and expertise on the development and implementation of operational functions to support accreditation and recognition products. Thomas most recently worked as the Director of Quality for Independent Health, a regional community-based health plan in Western New York. She has decades of experience in all aspects of QM/QI, as well as process development, regulatory oversight, accreditation and performance management. Thomas has been widely successful in building strategic plans for clinical integration and quality performance across many systems. Dr. Thomas holds a B.A. in Biology, as well as a Masters and Doctorate in Public Health with a concentration in Epidemiology.

 

Continuing Education

This course grants 1.0  required Continuing Education Unit (CEU) points for PCMH Certified Content Experts.

This is a non AMA PRA Category 1 CreditTM, ANA CNE, ACPE activity.
