Continuous QI & DEI – Better Knowing and Managing Patients 

How much we know about our patients determines the quality of care we provide. Social determinants of health (SDOH) and other factors make up 70% of patient’s overall outcomes. Patient Centered Medical Home (PCMH)- Knowing and Managing Patients concept can be used as a foundation to incorporate SDOH, diversity, equity, and inclusion components in with practice workflows, and EMRs to drive reports and offer focus areas for quality improvement.  

 During this session we will use PCMH as a foundation to introduce diversity, equity, and inclusion through knowing and managing patients, and continuous quality improvement. 

What You Will Learn

After successful completion of this activity, learners will be able to: 

  • Realize criteria in PCMH Knowing and Managing Patients.  
  • Utilize a comprehensive health assessment to gather SDOH. 
  • Identify how Knowing and Managing patients can impact whole person care and drive positive outcomes related to health equity.  
  • Leverage the EMR for workflows, and data sharing for continuous QI and education.
