Self-Paced Recorded Date: Wednesday, April 12, 2023; 2:00-3:30pm ET This training will use PCMH as a foundation to introduce diversity, equity, and inclusion through knowing and managing patients, and continuous quality improvement.

2. 2023 Town Hall: Future Updates

Live Webinar Wednesday, August 23, 2023; 1pm-2pm (ET) During this event, we will discuss publication changes, PCMH guidance updates and provide ample opportunity for Q&A.

3. Harnessing the Power of Diagnostic Data to Support Care Management, Quality Improvement and Cost ...

Live webinar September 13, 2023 2pm -3:30 pm ET During this session we will use PCMH as a foundation to introduce diversity, equity, and inclusion through knowing and managing patients, and continuous quality improvement.

Live Webinar November 14, 2023 1pm - 3pm ET This webinar will help PCMH practices understand the larger goals of Annual Reporting and prepare for annual submission under the Patient-Centered Medical Home program.

6. Race, Ethnicity, Sexual Orientation, Gender Identity, Income, and Language in Patient Registration

Live Webinar This session has been rescheduled to February 22, 2024- 1pm-2:30 pm Educational training for front desk staff and registration to increase competence and confidence in discussing, reviewing, and completing patient demographic data.

7. PCMH: Strategies to Improve Transitions

Self-paced Course Expires: 12/31/2023 This training will review case studies and examples of successful care transitions implemented by organizations in multiple settings including rural and urban, large organizations and solo practices.

8. SOGI Webinar - LGBTQ+ Health Care

Self-paced Expires: December 31, 2023 This course will examine health disparities, unique health needs and experiences of LGBTQ+ individuals.

9. Defining and Actualizing the ROI of being a PCMH

Self-paced Course Expires: December 31, 2023 This course is a discussion about the expected return on investment for becoming a PCMH. Faculty will highlight different ways to define ROI through the eyes of various stakeholders.

10. Aligning Standardized Measurement

Self-paced Course Expires: December 31, 2023 This training will discussion about how to prepare your PCMH Transform and sustain submissions.

11. Including Patient Priorities into QM with PDO

Self Paced Expires: December 31, 2023 This training will discuss how to implement the person-driven outcomes approach in clinical care, as well as the benefits of the approach for individuals and their care team.

12. Gender Harmony

Self-Paced Expires: November 14, 2024 Panel members will clearly articulate why disambiguating sex and gender identity data in clinical systems is critically important to improving clinical outcomes and ensuring data quality.
