2024 LTSS Best Practices Academy Membership

The LTSS Best Practices Academy is an interactive forum for professionals to discuss strategies for coordinating quality LTSS programs. The academy fosters informative discussions and enriching information exchanges. For more information about the Academy, visit our website here. 

Since its launch in 2017, the Academy has continuously improved its benefits based on member feedback. The 2024 Academy features live, interactive education sessions and access to recordings and materials if unable to attend a live event. There is an Academy newsletter that highlights industry news, suggested reading, educational opportunities, and an overview of a case management practice standard. In addition, the Ready Reference resource index is an exclusive benefit. 

Membership for community-based organizations is $475. Health plans, consultants, and all other organizations is $1180. The annual membership fee covers one legal entity, however multiple individuals within that entity may participate under that membership.

Important note: To ensure timely access to the Academy, delegate registration to one person at your organization. This person will receive instructions and the code to be utilized by subsequent registration of employees of the same legal entity. Instructions and the code are sent upon completion of the registration transaction.

3. 2024 Measurement & Outcomes | Quality Improvement CM-LTSS 5 -LTSS 2

This session delves into the quality improvement cycle and types of measures. Strategies for a successful survey and frequently asked questions regarding measurement, outcomes, and the standards.

4. 2024 Person-Centered Assessment

Panelists highlight applicable case management practice standards, NCQA standards, survey tips, and real-life scenarios focusing on person-centered assessment concepts.

5. 2024 Person-Centered Care Planning & Monitoring
