This training is a combination of NCQA's Introduction of PCMH: Foundational Concepts of the Medical Home and NCQA's Advanced PCMH: Mastering the Medical Home Transformation.

ABOUT THIS SELF-PACED TRAINING

NCQA'S INTRODUCTION TO PCMH ONLINE COURSE: FOUNDATIONAL CONCEPTS OF THE MEDICAL HOME

(Runtime: 5 hours)

This program is designed as an introductory course for anyone interested learning the fundamentals of the PCMH Recognition program. The course includes 12 modules on the basic topics of PCMH including the core attributes of patient-centered medical homes, the required evidence and determining how to present it as well as the processes and procedures that demonstrate transformation into the medical home model.

NCQA'S ADVANCED PCMH ONLINE COURSE: MASTERING THE MEDICAL HOME TRANSFORMATION

(Runtime: 3 hours)

The NCQA PCMH Recognition program plays an integral role in practice transformation and improving the quality of health care. This advanced course is intended for individuals who have applied NCQA PCMH Recognition requirements in the practice setting. It examines the PCMH standards and the assessment process. It takes a hands-on approach to each PCMH Concept, and a deeper dive to explore the key characteristics of a successful medical home.

This course will review evidence to support the core and elective criteria. It will also focus on advanced topics such as the Distinction Modules, electronic Clinical Quality Measures, and how to receive autocredit. 

WHAT YOU WILL LEARN

NCQA's Introduction to PCMH Online Course: Foundational Concepts of the Medical Home Objectives

At the conclusion of the program, participants will have the knowledge they need to:

  • Identify the core attributes of patient-centered medical homes.
  • Identify required evidence and determine how to present it.
  • Describe processes and procedures that demonstrate transformation into the medical home model. 
  • Outline the roles of the NCQA Representative and Evaluator and CCE.
  • Examine the challenging requirements in a variety of practice environments.
  • Outline the features and functions of the Q-PASS system.

 

NCQA's Advanced PCMH Online Course: Mastering the Medical Home Transformation Objectives

At the conclusion of the program, participants will have the knowledge they need to:

  • Discuss transfer credit and distinctions.  
  • Explain how elective criteria build upon core criteria within a competency.  
  • Determine a strategy for preparing a practice for the three (3) virtual check-ins and begin to create a transformation plan. 
  • Describe the types of evidence a practice can provide to meet the intent of a criterion, competency and concept.

WHO SHOULD ATTEND

This program is designed for any health care professional, health care decision maker or consultant interested in getting the inside perspective directly from expert faculty from NCQA in evaluating a primary care practice and their ability to become Recognized as a Patient-Centered Medical Home by NCQA.

NCQA's Introduction to PCMH Online Course: Foundational Concepts of the Medical Home

In support of improving patient care, the National Committee for Quality Assurance is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC) to provide Interprofessional Continuing Education for the healthcare team. 

This educational activity is approved for: 7.75 AMA PRA Category 1 CreditsTM, ANA CNE, and  ACPE activity.

The assigned universal program number: JA0004597-0000-23-007-H04-P. Upon successful completion of this program (attending the full session and completing a program evaluation), participants will access CPE Monitor on the ACPE website to locate and track their CPE statement of credit.

This program grants 8.0 Continuing Education Unit (CEU) points for PCMH Certified Content Experts.
