Person-Driven Outcomes: Incorporating Patient Priorities into Quality Measurement OnDemand

In July 2021, NCQA added the person-driven approach to the guidance for PCMH criterion CM 06: Patient Preferences and Goals, as a structured approach to eliciting and documenting patient preferences. During the Certified Content Expert webinar on January 25, 2022, we reviewed the person-driven outcomes approach, including methods and insights to ensure practices collect and document an individual's prioritized goals accurately. The OnDemand will also discuss how to implement the person-driven outcomes approach in clinical care, as well as the benefits of the approach for individuals and their care team.

  • Describe how outcomes identified by the individual (or caregiver) as important can be used for care planning and quality measurement.
  • Crosswalk the person-driven outcomes approach with existing PCMH criterion to elicit and document patient preferences.
  • Implement strategies to assist practices in using the person-driven outcomes approach, including tips to assess goal appropriateness and accurate documentation of a patient goal.

Caroline Blaum, MD, MS

Dr. Caroline S. Blaum joined NCQA from NYU Langone Health where she was the Diane and Arthur Belfer Professor of Geriatric Medicine and Director of the Division of Geriatrics and Palliative Care from 2012-2020. At NYU, Dr. Blaum maintained an active clinical practice and supervised clinical education for medical trainees in geriatrics and palliative care.  She led an extensive reach research program in translational research that was supported by the NIA, AHRQ, PCORI, and The John A Hartford Foundation concerning models of care for vulnerable populations, multiple chronic conditions, frailty, and diabetes in older adults. Prior to going to NYU, Dr. Blaum was Professor and Assistant Dean for Population Health at the University of Michigan Health System where she led UM’s experiences in Medicare managed care, ACO’s and PCMH. Dr. Blaum has been active in national policy work related to delivery system redesign and quality of care for older adults and people with multiple chronic conditions and has worked with the American Geriatrics Society, the AMA PCPI, and NQF.

Brianna Clifford, MPH, PCMH CCE

Bri Clifford is a Senior Policy Manager in NCQA’s Recognition Programs Policy and Resources Department. On this team she supports recognition programs through the creation of resources, maintenance of programs and education of both internal and external stakeholders.

Before joining NCQA, she worked with the United States Department of Health and Human Services Office of Emergency Management. There she helped create a GIS mapping tool used to track patient movement to HHS staffed medical facilities in response to natural disasters. She also spent three years managing the clinical side of specialty practices and volunteering as an EMT in her community.

Bri holds a BS in Biology from Virginia Tech and an MPH in Epidemiology from George Mason University.

Lee A. Jennings, MD, MSHS

Lee Jennings is the Chief and Associate Professor, Reynolds Section of Geriatric Medicine at the University of Oklahoma Health Sciences Center. She is a board-certified geriatrician and health services researcher whose work focuses on improving models of care delivery for dementia, aligning care received with patient health goals, and improving the competency of health professions students to provide high quality geriatric care. Ms. Jennings is the Principal Investigator for 3 statewide grants funded by HRSA, ACL, and the Reynolds Foundation focused on geriatric health care workforce development and community health education for older Oklahomans and their family caregivers. She is a co-investigator on research funded by PCORI, NIA, and the John A. Hartford Foundation and co-chairs the Women in Geriatrics Section for the American Geriatrics Society. Clinically, Ms. Jennings specializes in comprehensive geriatric assessment and cognitive evaluation and sees patients at the OU Physicians Senior Health Clinic and the Oklahoma City VA Health System.

Aanand D. Naik, MD

Aanand D. Naik, MD, is Professor and Nancy P. & Vincent F. Guinee, M.D., Distinguished Chair and Director of the University of Texas Health School of Public Health & Consortium on Aging in Houston, TX. Dr. Naik is also an investigator at the Houston Veterans Administration Center for Innovations in Quality, Effectiveness, and Safety (IQuESt) and co-director of the Southeast Texas Geriatrics Workforce Enhancement Program (SETx-GWEP).

Integrating these roles, his research program uses population health systems interventions to improve the healthcare decisions and outcomes of older adults with complex and multimorbid conditions. In particular, he is a co-leader of a national collaboration for Patient Priorities Care, an innovative model of multimorbid care: His research program includes randomized clinical trials, implementation science studies, and primary data collection with continuous research funding since 2006. These studies have produced >200 peer-reviewed manuscripts, commentaries, and book chapters.
