Knowing and Managing Your Patients “Deconstructed”

NCQA’s Knowing and Managing Your Patients is the largest of the NCQA Recognition Concepts and has the most Core and Elective criteria. This session will break down the most challenging KM criteria into understandable components of an overall comprehensive assessment and discuss how to use data to identify and segment populations and drive decision making for services offered to those populations.
