Join the discussion on KM07, where we reflect on SDOH and how the pandemic complicates this already critical aspect of health outcomes. We’ll look at ways to utilize data to connect patients with community resources and track outcomes. We’ll also take a brief look at capturing information on vulnerable populations and implementing a quality improvement plan. This is a deep dive into how patients are affected by social risk and how your clinic can track and guide them to better health outcomes.
