Using Health IT to Enhance Communication Between Patients and Providers

EHRs are evolving to keep up with the quickly developing IT platforms built to enhance care coordination and communication with patients and families. In this webinar, we will discuss what’s available in most EHRs to better inform patients about care plans and provide access to full chart information, and to enhance communication between the care team, provider and patient or family. Topics include sharing progress notes, use and set-up of text messaging, implementing proxy portals for populations with confidentiality concerns and use of CommonWell and Carequality.

Targeted Digital and Telephonic Messaging as Effective Support and Engagement Tools During the COVID-19 Pandemic

Once NY Governor Andrew Cuomo instituted NY PAUSE, Rapha Family Medicine, like most small, community-based practices, was left without guidance or resources to support its population, especially the most vulnerable. Understanding how small primary care practices used patient portals, phone calls and text messaging to communicate, engage with, manage and support their patient populations is critical—not just for care during the pandemic, but for ongoing transition and continuity of care afterwards. This presentation explores the critical importance of follow-up messages and/or virtual meetings with patients and their family members to care plan implementation, ongoing support and reevaluation.

Using Health IT to Enhance Communication Between Patients and Providers

Samantha Sawdon, PCMH CCE
Ms. Sawdon is the principal consultant at Synergistic Strategies LLC. She is an experienced professional with the ability to manage operations, implement change, processes and people. She has expertise in managing enterprise-wide health information technology projects, including coordination of stakeholder and vendors, project research, specifications, and deliverables, implementation strategies, user application definition and workflow design, validation processes, quality assurance, and support. She has strong leadership skills to focus her teams on project timelines and deliverables.

Prior to starting her company, Sawdon worked as a Managing Consultant for Massachusetts eHealth Collaborative and was a Practice Manager for major hospitals in the Boston area. She managed the Senior Health Care practice at Massachusetts General Hospital, a Multi-Specialty practice at St. Elizabeth’s hospital and the Cardiology and Vascular Surgery Departments at Boston Medical Center.  Sawdon holds a degree from the University of New Hampshire and has been a PCMH CCE for five years.

Frances Ilozue, MD, MPH, PCMH CCE 
Dr. Ilozue is a Primary Care Physician, originally from Nigeria in West Africa. She graduated from State Medical Academy, Saint Petersburg Russia in 1996. She was at University at Buffalo Internal Medicine/Pediatric Residency from 2002–2004, and later completed Family Medicine Residency at Conemaugh Memorial Medical Center Johnstown, Pennsylvania, in 2007. Ilozue became an NCQA PCMH Certified Content Expert in 2016. She also has an MPH degree from the George Washington University. 

Ilozue established Rapha Family Medicine in Buffalo, New York, a comprehensive primary care practice serving the Buffalo community since 2007. She strongly believes that, every individual should be educated and empowered to take charge of their health through a strong patient-doctor partnership. She has partnered with other local physicians committed to improving the health of and children and adults in Western New York.
