CCE Quarterly: Aligning Standardized Measurement within a PCMH

 As Recognized practices adopt and maintain a culture of quality improvement, different stakeholders (e.g., health care organizations, payers, regulators, employers) might be interested in specific measures. Standardized measures not only provide future benchmarking opportunities, but can reduce the burden of reporting similar or derivative measures. NCQA has identified an expanding menu of standardized measures that meet PCMH Recognition requirements and align with national quality programs such as MIPS. Join NCQA faculty and CCEs in a discussion about identifying quality measures that can meet a variety of reporting needs and improve patient care.

Bri Clifford, MPH, PCMH CCE

Bri Clifford is a Senior Policy Manager in NCQA’s Recognition Programs Policy and Resources Department. On this team she supports recognition programs through the creation of resources, maintenance of programs and education of both internal and external stakeholders.

Before joining NCQA, she worked with the United States Department of Health and Human Services Office of Emergency Management. There she helped create a GIS mapping tool used to track patient movement to HHS staffed medical facilities in response to natural disasters. She also spent three years managing the clinical side of specialty practices and volunteering as an EMT in her community.

Bri holds a BS in Biology from Virginia Tech and an MPH in Epidemiology from George Mason University.

Nathan Dolan, MHA, PCMH CCE, SSGB, LPC

Nathan Dolan serves as Senior Lead Advisor, Lean Facilitator Healthcare at Purdue Healthcare Advisors (PHA). In this role, he teaches and advises hospitals and healthcare agencies throughout Indiana and the Midwest to adopt lean methodologies. Prior to his position at Purdue University, Nathan spent more than eight years working in the mental health and ambulatory care setting across northern Indiana in the areas of Operations, Quality, Marketing and Outreach, and Patient Relations. Nathan has implemented various Lean and Six Sigma projects throughout his time in healthcare helping improve processes for submitting claims, increasing patient access, improving call center operations, achieving clinic PCMH recognition and recertification, IT/EHR training and implementation, and improving CMS quality metrics. Nathan earned his bachelor’s degree in Anthropology and Business Administration from Longwood University and his master’s degree in Healthcare Administration from Valparaiso University. He is a Patient-Centered Medical Home Certified Content Expert (PCMH CCE), Six Sigma Green Belt (SSGB), Lean Daily Improvement Facilitator (LDIF), and Lean Practitioner Certified (LPC).

Emily Morden, MSW

Emily C. Morden, M.S.W., is the Director of Electronic Measurement Strategy in the Performance Measurement Department at NCQA. Ms. Morden leads projects on the development and maintenance of health care performance measures primarily addressing geriatric and behavioral health care. Her areas of interest and expertise include health care quality measurement and improvement, leveraging health information technology for measurement, health care data analytics, delivery system transformation, integration of behavioral health care, medical social work services and care for vulnerable older adults. 




In support of improving patient care, the National Committee for Quality Assurance is jointly accredited for Continuing Medical Education (ACCME), the  American Nurses Credentialing Center (ANCC), the American Academy of Physician Assistants (AAPA), the American Psychological Association (APA), and the Association of Social Work Boards (ASWB) to provide Interprofessional Continuing Education for the healthcare team.
