CCE Quarterly: Reflecting and Getting Back on Track with Patient Care after a Pandemic or Natural Disaster OnDemand
Adversity often sparks ingenuity, and this year has been full of challenges and adversity of PCMH practices across the country. From natural disasters like fires, hurricanes and blizzards to the rise of COVID-19, PCMH practices have had to innovate and adapt. Join NCQA leadership and CCE faculty for a round table discussion about how to prioritize patient-care during challenging times and how to stay on track with PCMH priorities. Faculty will review how PCMH practices readjusted quality improvement activities during this past year.

At the conclusion of this course, you'll be able to:
- Describe how the PCMH model builds the scaffolding required to support practices in times of adversity.
- Identify at least four examples of how the practices spotlighted in this course adapted and changed their standard PCMH processes to address emerging needs and challenges.
- Outline a strategy for how to incorporate and formalize temporary changes in policies into your long term PCMH operations that continue to ensure the delivery of safe, effective patient centered care.
- List at least four tips or lessons learned that should be considered and addressed when developing a PCMH Preparedness plan.
Karen Taubert, RN, BSN, MBA, PCMH CCE
Ms. Taubert possesses 30-plus years of progressive clinical and administrative leadership experience in acute, home health, nursing facility, and safety net settings. She is currently a Senior Consultant for Comagine Health, a position in which she provides a variety of technical assistance activities related to the company’s medical home consulting engagements. Before joining Comagine Health, Ms. Taubert served as the Chief Nursing Officer for an FQHC, Su Clinica Familiar, in Texas. In that role, she directed the work of more than 100 employees, project managed a PCMH recognition project, and oversaw a variety of performance improvement and accreditation projects. Ms. Taubert previously held positions as the Director of Nursing for two other organizations and served in clinical positions in oncology and medical-surgical settings. Ms. Taubert has been a PCMH CCE since 2013 and was a 2020 – 2021 PCMH CCE Quality Award Winner.
Amy Tennant, PCMH CCE
Amy Tennant is a Policy Manager in NCQA’s Recognition Programs Policy and Resources. In this role, she creates and maintains PCMH resources, manages product updates and provides policy support and expertise to customers, PCMH CCEs and NCQA Evaluators. She started at NCQA in the Product Development department where she supported the development of recognition programs including the redesign of the PCMH standards for 2017, Patient-Centered Specialty Practice School-Based Medical Home and the creation of the Oncology Medical Home, the first recognition program created for a specific medical specialty. Before starting her career at NCQA, she was an intern at MedStar Georgetown University Hospital where she worked on reducing medication reconciliation errors and incidence of outpatient falls. She obtained her Six Sigma Black Belt and holds a BS in Healthcare Management and Policy from Georgetown University in Washington, DC.
In support of improving patient care, the National Committee for Quality Assurance is jointly accredited for Continuing Medical Education (ACCME), the American Nurses Credentialing Center (ANCC), the American Academy of Physician Assistants (AAPA), the American Psychological Association (APA), and the Association of Social Work Boards (ASWB) to provide Interprofessional Continuing Education for the healthcare team.
