Presenters will look at case management standards of practice, discuss the NCQA standards, and lead examination of a case study using SMART goal setting.

The NCQA LTSS standards that pertain to person-centered care are CM-LTSS 3 (A, B, C) or LTSS 1 (E, F), depending on your organization's type of recognition.
