Presenters will look at case management standards of practice, discuss the NCQA standards, and examine case studies focused on care transitions from CBO and Health Plan perspectives.

The NCQA LTSS standards that pertain to person-centered care are CM-LTSS 4 (A - C) or LTSS 3 (A - C), depending on your organization's type of recognition. Please take a moment to review the standard in advance of the session.
