NCQA’s Advanced PCMH Program: Mastering the Medical Home Transformation
This advanced course is intended for individuals who have applied NCQA PCMH Recognition requirements in the practice setting. It examines the PCMH standards and the assessment process. Participants will take a deeper dive to each PCMH Concept and explore the key characteristics of a successful medical home in this hands-on program.
Faculty and participants will discuss evidence to support the core and elective criteria. The program also focuses on advanced topics such as the Distinction Modules, transfer credit and program benefits. Attendees will participate in facilitated activities that demonstrate how earning elective credits builds upon the Core criteria of a Competency. Our faculty will incorporate topics on behavioral health integration, the importance of assessing the social determinants of patient health, engaging with patients and the community, and continuous quality improvement.
Learning Objectives
At the conclusion of the program, participants will have the knowledge they need to:
- Discuss transfer credit and distinctions.
- Explain how elective criteria build upon core criteria within a competency.
- Examine opportunities and approaches to succeed in the PCMH model of care.
- Determine a strategy for preparing a practice for the three (3) virtual check-ins and begin to create a transformation plan.
- Describe the types of evidence a practice can provide to meet the intent of a criterion, competency, and concept.
This training will include both on-demand (recorded) content and live webinars.

Jennifer Anglin, MS, CHES, PCMH CCE
Jennifer Anglin is a Policy Manger of Recognition Policy at NCQA. Her responsibilities include creation and maintenance of PCMH resources and providing support to practices going through transformation or annual review. Prior to NCQA, Jennifer worked for Purdue University by assisting practices throughout the state of Indiana on quality improvement and research projects as well as supporting them to meet Medicaid’s Promoting Interoperability program and Medicare’s MIPS program. Jennifer’s professional interests include health policy, research, and education. She has a particular interest in social determinants of health and the behavioral health aspect of Patient-Centered Medical Homes.
Jennifer holds a BA in secondary education from Valparaiso University and a master’s degree in health science from Indiana State University. Jennifer is a Certified Health Education Specialist and has been a NCQA Patient-Centered Medical Home Certified Content Expert (PCMH CCE) since 2013.
William F. Tulloch, MA, PCMH CCE
Bill is NCQA’s Director in the Quality Solutions Group and has been with NCQA since 1997. In his role, Bill manages the operations of several government contracts aimed at promoting the Medical Home concepts and works with large health systems that are Recognized or seeking Recognition by NCQA. Bill is a PCMH Certified Content Expert with NCQA.
Prior to this position, Bill was the Director of Customer Resources, providing educational assistance to organizations seeking to improve the quality of care and service they provided. Bill was also previously Director of the Privacy Certification for Business Associates (PCBA) program, a joint offering of NCQA and the Joint Commission on Accreditation of Healthcare Organizations. Bill helped develop both the PCBA and other new assessment programs in the health care field and improving existing NCQA products as part of the Product Development team. Earlier in his tenure at NCQA Bill served as a Senior Accreditation Manager participating in and coordinating surveys for the MCO, CVO, and MBHO programs. In addition to his other work, Bill has been a frequent faculty member for educational presentations for NCQA, and is a member of the internal team that developed and maintains the Interactive Survey System.
Bill attended Williams College for his undergraduate work, and has a Master’s in Economics from the University of Maryland. For seven years prior to joining NCQA, Bill worked with a health care consulting firm, HTA (now Covance), in Washington, DC, helping medical product manufacturers work in the managed care setting. During his undergraduate years, Bill co-authored two papers on the psychology of juror decision-making, based on original research.
Fatin Yousufzai, MSHP, PCMH CCE
Fatin Yousufzai is a Manager of Recognition Programs Policy at NCQA. Her responsibilities include the creation and maintenance of PCMH resources and providing support to practices going through transformation or annual review. Prior to becoming a manager in Recognition Policy, Fatin was a Policy Analyst on the Performance Measurement team at NCQA, where she supported HEDIS measure development, maintained derivative HEDIS publications (QRS, ACO), and produced HEDIS Value Set Directories (VSD).
Fatin earned her B.S. in Health Sciences from Howard University and M.S. in Health Policy from George Mason University.
In support of improving patient care, the National Committee for Quality Assurance is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), the American Nurses Credentialing Center (ANCC), the American Academy of Physician Assistants (AAPA), the American Psychological Association (APA), and the Association of Social Work Boards (ASWB) to provide Interprofessional Continuing Education for the healthcare team.
This activity was planned by and for the healthcare team, and learners will receive 3.5 Interprofessional Continuing Education (IPCE) credit for learning and change.
This educational activity is approved for: 3.5 AMA PRA Category 1 CreditTM, ANA CNE, APA, ACPE and ASWB ACE.
