This OnDemand webinar is designed to help PCMH practices understand the larger goals of Annual Reporting and prepare for annual submission for reporting year 2021 under the Patient-Centered Medical Home program. 

Faculty will highlight ways to demonstrate sustained recognition status through maintenance and continuous quality improvement efforts using data that a practice is already collecting internally and for other sources (e.g., state initiatives). Timelines for deliverables and performance expectations will be key discussion items.

Target Audience

This OnDemand webinar is geared towards individuals with one year of experience, or more; collecting, reporting, and acting upon annual reporting data; or those who need a refresher. This program is designed for individuals in the practice that work with patients, and with PCMH recognition.

At the conclusion of the webinar, participants will be able to:
•    Outline the Annual Reporting requirements for PCMH recognition.
•    Describe the Annual Reporting process; including timelines, submission and audit.
•    Interpret Annual Reporting requirements for reporting year 2021. 
•    Recognize important features and functions of Q-PASS to submit for Annual Reporting.

Bri Clifford, MPH, PCMH CCE

Bri Clifford is a Policy Manager in NCQA’s Recognition Programs Policy and Resources Department. On this team she supports recognition programs through the creation of resources, maintenance of programs and education of both internal and external stakeholders.

Before joining NCQA, she worked with the United States Department of Health and Human Services Office of Emergency Management. There she helped create a GIS mapping tool used to track patient movement to HHS staffed medical facilities in response to natural disasters. She also spent three years managing the clinical side of specialty practices and volunteering as an EMT in her community.

Bri holds a BS in Biology from Virginia Tech and an MPH in Epidemiology from George Mason University.
 

This webinar provides maintenance of certification credit (2 points) under "other"  for PCMH CCEs.  This is a non AMA PRA Category 1 CreditTM, ANA CNE, APA, ACPE and ASWB ACE activity.
