PCMH Recognition Annual Reporting: Continued Success
Join us for a live webinar designed to help PCMH practices understand the larger goals of Annual Reporting and prepare for annual submission for reporting year 2021 under the Patient-Centered Medical Home program.
Faculty will highlight ways to demonstrate sustained recognition status through maintenance and continuous quality improvement efforts using data that a practice is already collecting internally and for other sources (e.g., state initiatives). Timelines for deliverables and performance expectations will be key discussion items.

• Outline the Annual Reporting requirements for PCMH recognition.
• Describe the Annual Reporting process; including timelines, submission and audit.
• Interpret Annual Reporting requirements for reporting year 2021.
