PCMH: Knowing and Managing Your Patients (KM)

A Look Inside PCMH Concept: Knowing and Managing Your Patients (KM)

NCQA's PCMH Recognition program has evolved to feature a set of six concepts that make up a medial home. Underlying these concepts are criteria (activities for which a practice must demonstrate adequate performance to obtain NCQA PCMH Recognition) developed from evidence-based guidelines and best practices.

This self-paced module focuses on the Knowing and Managing Your Patients (KM) concept. KM requires the practice to collect patient information to ensure a comprehensive and holistic view of the patient's history, needs, and preferences, and then uses the information to deliver appropriate evidence-based and patient-centered care. During this online activity participants will learn about KM's core and elective criteria and view example documentation that meet their intent.

For an overview of all 6 PCMH Concepts, please refer to the "A Look Inside the 6 PCMH Concepts: A Comprehensive Overview" course.
