Following recent reports from the World Health Organization (WHO) and the Centers for Disease Control and Prevention (CDC), NCQA has decided to cancel our seminar offerings in May in FL. 

Gov. Ron DeSantis directed State Health Officer and Surgeon General Dr. Scott Rivkees to declare a public health emergency in Florida on March 1, 2020.  Our top priority is the health and safety of our attendees.

NCQA's Introduction to PCMH Online Course
We have an on demand course that you may complete at your own pace from any internet connection. This course covers the same content as the in-person seminar and also meets part of the eligibility requirements for learner pursuing their PCMH CCE credential.

A Look Inside the 6 PCMH Concepts: A Comprehensive Overview
This self-paced, on-demand training provides an overview of the 6 PCMH concept areas. Learners may also choose to explore individual concepts. While this course does not review specific recognition standards, it is a good primer for those new to PCMH Recognition.

Learn successful strategies for achieving and sustaining PCMH recognition for medical homes from any care setting. 


NCQA's Introduction to PCMH Program: Foundational Concepts of the Medical Home

This program is an in-depth guide to NCQA Patient-Centered Medical Home (PCMH) requirements and the process for achieving PCMH Recognition. The Medical Home model emphasizes a practice-wide commitment to transformation by demonstrating key features such as population health management, care coordination and continuous quality improvement.

NCQA requirements are positioned to help clinicians qualify for the Merit-Based Incentive Performance System (MIPS) Improvement Activities and supports meeting the other MIPS categories under MACRA.

Target Audience

This program is designed for any health care professional, health care decision maker or consultant interested in getting the inside perspective directly from expert faculty from NCQA in evaluating a primary care practice and their ability to become Recognized as a Patient-Centered Medical Home by NCQA. 

Learning Objectives

  • Explain the core attributes of patient-centered medical homes.
  • Outline required evidence and best methods for presenting it to NCQA.
  • Know the distinct roles of the NCQA Representative, Evaluator and PCMH CCE. 
  • Identify criteria relevant to different practice types to establish a pathway towards Recognition.  
  • Outline the important features and functions of the Q-PASS system.
