Learning in Action: Implementing a Primary Care Telehealth Response During COVID-19
During this course, attendees learn how the Care Transformation Collaborative of Rhode Island (CTC-RI) helped primary care practices respond to telehealth implementation challenges through a three-pronged “learning in action” approach. Areas of focus include development and implementation of needs assessment surveys; utilization of results to inform telehealth webinar topics that align with proposed PCMH/PCSP Telehealth Distinction Requirements; and development and implementation of a primary care 12-month telehealth learning collaborative that let practices test/use telehealth services to improve care for patients with chronic conditions. Hear from a primary care practice that participated in this initiative and get sample tools and resources to apply in your primary care practice setting.

Susanne Campbell, RN, MS, PCMH CCE
Susanne Campbell, RN, MS, PCMH CCE, has a broad range of clinical and administrative experiences in outpatient primary care practices, community health centers, home care and long-term/assisted living care. In her current position at Care Transformation Collaborative of Rhode Island, she has supported expansion of adult and pediatric primary care practices earning NCQA Recognition, improving clinical and customer experience outcomes and decreasing total costs of care. Ms. Campbell has worked with primary care practice teams and health plans to apply a population health approach to strengthen clinical and community links and improve health equity. She received her Masters in Gerontology Nursing from the University of Rochester, her Masters in Health Service Administration from Salve Regina University and her BSN from Mount Saint Mary College.
Sue Dettling, BS, PCMH CCE
Sue Dettling is a PCMH content expert, Practice Facilitator and Telehealth Project Manager at Care Transformation Collaborative of Rhode Island. Prior to joining CTC-RI, she worked with primary care practices in RI to help them earn NCQA PCMH Recognition. In addition, she has provided technical assistance and quality improvement coaching to primary care and specialty practices, helping them meet CMS program requirements, including the Quality Payment Program, Transforming Clinical Practices Initiative and the EHR Incentive program. Ms. Dettling is Project Manager for the RI telehealth project and practice facilitator leading work with FQHS, pediatric and adult primary care practices and community health teams. She received her BS in Health Policy and Management from Providence College, and has maintained her NQCA CCE since 2014.
Marna Heck-Jones is IT EHR Coordinator at Anchor Medical Associates. Prior to that position, she helped develop and implement systems and workflows to improve the provider and patient experience and report quality data in a variety of health care settings in Oklahoma, Oregon and throughout New England.
This live course grants 1.0 Continuing Education Unit (CEU) points for PCMH Certified Content Experts.
